Your application will remain on file for 180 days. Examples of duties are managing the central supply area for the Nursing Department. Medical Records duties: 35% Maintains the master IHN database system for the entire facility, which contains essential demographic and medical information for all residents in the facility and transcribes all physicians’ orders to database. This information is used as the resource for all billing at the facility. Makes data entries daily and prepares a daily census report which is sent to the VA of Jurisdiction. This report determines financial reimbursement for the facility from the VA system, which directly relates to ensuring the budget projections are met. Updates information in this system when any changes are made relative to the resident while at the facility. Using the information in this system, prepares reports that are sent to various Department Heads in the facility and outside sources to include State Headquarters and VA facilities. Compiles, calculates and prints demographic reports for Administrator of the facility. 10% Files pertinent medical information on the active medical record located on the nursing units. This includes filing ancillary services reports such as laboratory reports, x-ray reports, consults, etc., as well as, any other information that is to be maintained on the record. Replenishes all forms that are required to be on the active medical record for use by staff members. Ensures that there is an adequate supply of form for use on each nursing unit. Removes information from the active medical record on the nursing units following an established protocol per facility policy. This entails pulling every unit record on a scheduled basis and checking every section of the record to determine if information needs to be removed based on a pre-determined timeline. 30% Processes all residents for admission to the facility. Prepares all required forms for the active medical record maintained on the nursing unit. Meets with the resident and/or authorized representative to complete all of the necessary forms including multiple HIPAA protected health care consent forms. Enters all of the resident information into the IHN database. Takes photos of the resident on admission and as needed. Prepares the Application for Health Benefits (10-10EZ) and Nursing Home Status Statement (F2) in coordination with the Veterans Service Officer. Initiates a Master Folder record to be maintained in the Medical Records department that is considered the legal medical record for the facility. All original documents are maintained in this folder. Prepares monthly physician’s orders, medication, MARS and treatment TARS. 20% Processes all discharges for the facility. This includes discharges to home, transfers to other facilities and deaths that occur while at the facility. When the resident is discharged, the active medical record from the nursing unit must be obtained and every form checked for completion before being placed in the official master folder. Any form that is not complete must be sent to the appropriate Department Head for correction. Since the record must be closed within a certain timeframe according to facility policy, a tickler system must be maintained to ensure the information is obtained timely. Prepares a discharge as required from the attending physician to place in the master folder. An audit must be performed to officially close the record to ensure that all of the required information is present in the event the record is requested from an outside source or subpoenaed for any legal action. 5% Prepares medical record information for release to the resident, an authorized representative or any other outside entity when an official request is made. Makes official copies of the record, as requested, and certifies the information is correct. Completes the necessary documents and obtains authorization from the Administrator to release the information in accordance with facility policy and applicable laws. Performs other related duties as required. Central Supply Admin Coordinator duties: Order all supplies, which includes medical supplies, clerical supplies and hygiene products as needed. Contact vendors for information regarding their products and coordinate with the facility purchasing agent. Maintain a perpetual inventory on all operating supplies. Completes requisitions and submits them to the purchasing agent. Must be able to lift up 75 lbs. Distributes supplies to the nursing units as requested. Monitors supply usage on all nursing units.
